2 What is the LD SAF? Collects data about how local services are working together to improve the lives of people with learning disabilities and improve the health and social care services available to support them. On a national level, this information will support action that improves outcomes from people with learning disabilities and their families. On a local level, information collected will help Clinical Commissioning Groups and Local Authorities identify the key issues they need to improve health and social care services.

3 What is the LD SAF? The Joint Health and Social Care Self-Assessment Framework helps areas check that people with learning disabilities and their families: Get the same life chances as other people such as having a job or being safe where they live. Get good access to health services to stop them becoming unwell when this could have been stopped.

4 What is the LD SAF? Three key areas; Staying Healthy- This includes getting a good service from the doctors, chemist, dentists, hospital and more. Being Safe- This is about being safe in lots of different places such as in your home, whilst in hospital or out and about on the bus. Living Well- This about making sure people are part of the what happens in their area such as festivals or going to the cinema.

5 Ratings This helps us decide how well we are doing. Red means we have a lot of work to do on something Amber means there is some work done but more is needed Green means we are doing really well.

14 Next steps Send our data to NHS England, who will add extra data about Oldham that is collected nationally and confirm or change our RED, AMBER or GREEN scores. Bring our final results back to the Learning Disability Partnership Board and to agree how we need to improve services, and how this will be included in the Joint Commissioning Learning Disabilities Strategy for Oldham, and Supported Living Commissioning Framework Make sure Get Oldham Working improves employment for people with learning disabilities Take our results to the Health and Wellbeing Board and discuss how well we are doing in Oldham, what our plans to improve services are, and how they can help us.
