3 Objectives  Learn what is involved in a Community Health Status Assessment (CHSA)  Understand the eight steps involved in a Community Health Status Assessment

4 Conducting the CHSA Answers:  How healthy is the community?  What does the health status of the community look like?

5 The CHSA collects quantitative information on:  Health Status  Quality of Life  Risk Factors Meets PHAB Measure 1.1.2 T/L Required Document 1.a.

6 Role of MAPP Steering Committee  Brainstorm existing sources of CHSA data  Critically think about the data that local public health system partners are willing to share  Determine if the data represent the status of the entire community & include entire system  Discuss whether the data have been used strategically to inform improvements

7  What are some existing sources of data that would be good to use for the CHSA?  Who owns those data?  Are there any data gaps? Group Discussion

8 Eight Step Process  Step 1: Create a Sub-committee  Step 2: Create a List of Indicators  Step 3: Collect Data for Selected Community Indicators  Step 4: Organize and Analyze Data  Step 5: Compile and Disseminate Results  Step 6: Create System to Monitor Indicators over Time  Step 7: Create a List of Challenges and Opportunities  Step 8: Share Results with Community

9 1. Create Subcommittee The sub-committee should include members who:  Have access to data  Can analyze data  Can create a system for managing data  Have an interest in data

Create List of Indicators Subcommittee members will brainstorm and select indicators that are important to the community  Should reference the vision and interest of MAPP team  Review 12 categories of data to decide which are most important (see next slide)  Discuss specific indicators or interest in each category

13 Brainstorm and Select Indicators  Indicators should be important to the community and meet meet PHAB requirements

Collect Data for Selected Indicators This may include both primary and secondary data collection systems (PHAB Measure 1.1.2 T/L RD 1.a.)  Conducting primary data collection should only be done if it is necessary and the community has the resources  There are many secondary data sources (see next slide)  Data collection does not have to be perfect  It is important to note the limitations of existing data and ways data collection can be improved in the future

Organize and Analyze Data  Analyze data by socioeconomic status, mortality rates, gender, age, race, ethnicity, geographic area, and other common population subgroups  Show trends over time  Compare results with peer communities, state averages and targets, and national targets such as Healthy People 2020

17  Can be presented in a written report, slide presentation, or online (PHAB Measure 1.1.2 T/L RD 2)  Results should be shared with community members and public health system partners  When sharing, provide information on how they can learn more about the MAPP process and provide feedback 5. Compile and Disseminate Results

18  Have you shared health status data with the community before?  What method did you use? Written report? Presentation? Online? Group Discussion

19 As your community progresses with its health improvement planning efforts, data will change over time (as least that is the hope)  Creating a system that is easily updated will allow the community to track these changes over time  Required in PHAB Measure 1.1.2 T/L RD 3  Requires clear definition of roles, leadership, coordination, and communication 6. Create a System to Monitor Indicators

20 Sub-Committee Should Make Decisions:  Frequency of data refresh  Quality of data  Comparison to peer, state, or national data  Need to modify or add indicators  Methods for maintaining data systems  Sophistication of technology used  Communication mechanisms that will support continuous use of the data system

21 In creating a list of challenges and opportunities (10-15), partners should examine the CHSA results in light of the following:  Does this health problem affect a large number of people?  Does it have serious consequences?  Does it show evidence of wide inequity between groups?  Is it susceptible to proven interventions?  Does the issue have broad implications over the long term?  Is there a potential for a major breakthrough in improving health outcomes? 7. Create a List of Challenges & Opportunities

22  Results can be distributing at meetings, online or through traditional or social media  Provide information about how people can learn more about the MAPP process and provide feedback on the assessment results – use to invite more participants to Phase 4 of the MAPP process  Different from step 5 in that this is the final CHSA product; step 5 was to obtain broader input into the assessment 8. Share Results with Community
