1 Remote Patient Monitoring (RPM) - Summary Eastern Health has an aging population, rural geography, with a continued rise of chronic disease conditions requiring a cost-effective approach to care delivery. EH partnered with Canada Health Infoway, and together with the provincial government, funded this project. RPM is an innovative program to support patients to self-manage their chronic disease at home using simple technology. The initial diagnoses to be monitored include COPD and CHF.
