REQUIREMENTS WWW.MYACTIVEHEALTH.COM/COVA ONLINE HEALTH ASSESSMENT Completion of an online health assessment no earlier than March 1, 2015 BIOMETRIC SCREENING Obtain a Physician Results Form and submit with measurements taken no earlier than April 1, 2013 Don’t forget to use your annual wellness benefits!

REMEMBER… Employee/Retiree and spouse must register separately to complete the online assessment and obtain a physician results form Requirements can be completed at any time during the year -- the the reward will be effective the first of the month after it’s received (fax or postmark date) If no online access, contact ActiveHealth at 866-938-0349 Enrollments effective on or after August 1, 2015, will not be eligible until July 1, 2016 (FY 2017)
