10. MOC Criteria Initially, to earn MOC, Faculty were required to attend at least 6 of the 9 LC calls, plus 1 orientation call UPDATE: Faculty can listen to up to 2 recordings of LC calls posted on the NIPA Virtual Toolkit to supplement their call attendance. Orientation call attendance does not count towards 6 LC calls and is not required for MOC. MOC Attestation Holly will send instructions on how to obtain and submit an attestation form to each site lead Once completed, each site lead will send their study team’s attestation forms to Holly Once the attestation forms have been submitted, each person will receive a confirmation email from MOC that both the form and points were received.

13. Post-Implementation Chart Reviews Each practice needs to generate a list of eligible patients. Patients who are eligible for the post- implementation chart review are adolescents aged 11-17 who were seen in your practice for any type of visit between August 1, 2015 and April 30, 2016. Identify all patients born between 5/1/1998 and 8/1/2004 who were seen in your clinic between 8/1/2015 and 4/30/2016 Alphabetize this list by last name and number them 1, 2, 3…. Save your patient list for use during the chart review Email the total number of patients (denominator) to hollyce@academicpeds.org (NOT the list of patients, just the number of patients on the list) hollyce@academicpeds.org Using this patient denominator, we will generate a random order of numbers that correspond to patients on your list.

14. Post-Implementation OSI & Staff Impact Survey Each practice will receive a site-specific link to complete these surveys in REDCap.
