12 I-9 Form Named for number of pages: 9 It is a U.S. Citizenship and Immigration Services Form. Verifies that each new employee is legally eligible to work in the United States. Both employees and employers fill out the form. The employee must also present the employer with acceptable documents evidencing identity and employment authorization. Form must be retained by the employer for at least (3) years after hiring date.
