1 SCIETT Florida Center for Inclusive Communities Action Plan Status 02/2010: Pensacola Year Four Develop infrastructure to sustain ongoing diverse community participation to market the value and expectation of employment of person with developmental disabilities and support four students to transition from school to successfully matched SCI work environment. Four Students will be identified to receive Discovery and PCP strategies to support their employment outcome Melinda Poe, ESE Escambia Co Schools Annette Zeeb, APD SE Liaison Tom Turner, DVR Liaison to APD January 2009 Completed Four students were identified. Two in Escambia, one in Santa Rosa and one in Okaloosa School system. Walton County did not have a student meeting our criteria. Provide resource information and education to ESE students in High Schools Kim Lonas, Walton County ARC Melanie Johnson, Santa Rosa ARC Lisa Bloodworth, Lakeview Center Julie Petrushak, Horizons Melinda Poe, ESE Escambia Co Schools April 2009 Completed at Transition Information Fairs Each team responsible person will collaborate with the transition coordinator of the respective county school district to facilitate the dates, locations among school personnel/event. Presentations will be generic to array of options and resources specific the employment expectations Identify SE Consultant to receive TA to provide the supports to each student. Annette Zeeb, APD Tom Turner, VR Liaison to APD January 2009 Completed As students identify their interest and preferences, Annette/Tom will process necessary documents, authorizations, and notify Provider and Brenda prior to SE Provider contact with student Get wait list specific to the Agency for Persons With Disabilities in the four counties between the ages of 18 – 22 years.
