2. 2 W LTI 1202 208 - Final report Sequence of events 1.Shipyard work in preparation for Arctic project. 2.Contract vessel being fitted with gear for project. 3.Welder and fitter working at height (10 meters). 4.Attempting to weld steel plate weight approx. 70 kg. 5.Plate slipped and dropped to lower deck. 6.Plate landed on deck one foot from worker. 7.Contractor Superintendent immediately stopped all work and held a safety stand down meeting with all workers involved with project.
