6 Community Wellbeing Service - Hastings Three levels of support are provided to patients to address the social and economic issues affecting their mental wellbeing: Level 1 - Brief intervention Level 2 - A one-off meeting to provide focussed support Level 3 - Case working with more intensive follow-up and on-going support

7 Benefits to patients Helps build confidence, engage with services and become more resilient Improves wellbeing by connecting with and exploring issues with a professional Enables access to a wide range of services that have a positive effect on health and wellbeing 88% have not visited their GP again about their mental or emotional wellbeing 75% have been supported to achieve their goals

8 Benefits to clinical teams Steady increase in the number of referrals from GP practices 80% stated that GP appointments for referred patients have reduced 100% stated that the CWS offers the type of non-clinical support required
