10 SP and SAP What is the status of the SP assessment in relation to the Single Assessment Process? What is the relationship of the resulting Support Plan to the SAP care plan?

15 Extra Care Housing and the SAP Range of partners –Housing authority – nomination rights –Housing Provider – accommodation, housing management and housing-related support –Supporting People – commissioners of SP services –Social Services – commissioners of care –Care provider – care and sometimes SP services –Health commissioners All have legitimate reasons for having a stake in the assessment of an applicant for an Extra Care scheme
