Commitment to working with Our Community “Each time a person uses his or her capacity, the community is stronger and the person more powerful. That is why strong communities are basically places where the capacities of local residents are identified, valued and used” Kretzman and McKnight, 1993 Our Community Health Board volunteers believe: Our CHB Board members have demonstrated their commitment to community with the countless hours that they volunteer, the community partnerships they seek and maintain and their focus to include community input into all that they do!

Assessing Our Community Beginning in 2004 all 5 CHB’s surveyed their communities: Differing styles (telephone/written/focus groups), differing content (independent/partnering with other groups) All based questions on Population Health Principals and the Determinants of Health Results formulated into “Community Needs Assessments”

Identifying Issues & Priorities The top issues from our surveys were reviewed and compared to other data and research. CHB’s considered, among other things: Are many people affected by the issue? Is there opportunity for us to reduce it further? Does the issue significantly impact on the quality of life? Are there other groups working on this issue? Will the problem become worse if it is not addressed? Is there public support for addressing the issue? Can we save money if the issue is addressed? Is the issue a priority with NSHPP?

CHB’s Considered : Any research and data on the issue. Best practice information. Population Health Framework. Multiple Strategies for addressing the issue. Strategies the CHB and AVH have taken within the past year. Community Demographics Recommendations were then formed for CHB’s and for AVH. May 2005--Health Plan’s submitted!

AVH Considers Recommendation’s Plans were reviewed, common themes were determined, Managers Workbook was created Summer/05. Information packages, including Response Workbook and copies of all plans, distributed to Leadership Team Aug/05. Presentation of planning cycle and review process to Leadership Team in September/05. Presentation to AVH Board Oct/05 re CHB Health Priority areas and CHB plan review process. Recommendations used in 2005 DOH & NSHPP Business plan development. Summary Report of manager’s responses prepared/ shared with Executive, Leadership & Staff etc May/ June 2006

Implementation of Recommendations AVH Used in 2005 business planning process. Currently used in Strategic Planning Process for 2006-2009 CHB’s District Education Day, Oct 2005 “Developing Action Plans” CHB’s develop board Action Plans to implement recommendations. Promote Wellness Initiative Funds to community groups working on CHB priority issues.

Monitoring Outcomes & Processes Health Planning Research Project to evaluate Health Planning approach and timelines. Wellness Initiative Fund process evaluation. Look for CHB partnership opportunities with AVH and with community groups. Communication between CHB’s and AVH. Celebrate activity related to CHB Priority Issues. Continue to work to increase awareness and understanding of CHB work across the district.
