Open Enrollment Period– Ending September 13, 2013 One plan for all members ALL HMO members must complete new enrollment forms. Current PPO members need only complete a new form if there are changes to your enrollment information. NOTE: No Primary Care Physician selection necessary. All forms MUST be turned in to PAYROLL by Friday, September 13, 2013. Open Enrollment Highlights

What Else Do I Need to Know? ID Cards –All Members will receive 2 new ID cards in the 9/30/2013 payroll –BE SURE TO DESTROY YOUR OLD ID CARDS ON OCTOBER 1, 2013. THEY WILL NOT WORK –Contact Allied at 866-455-8727 if you need an additional card. –One card for both medical and Rx Claims –All bills for services prior to 10/1/2013 continue to go to BCBS. Call BCBS with questions on these claims. –Use your New ID Card on or after 10/1/2013 –Be sure your doctors record the new claim filing instructions. –Allied processes all claims with dates of service of 10/1/2013 and after. NOTE: ALLIED PROCESSES ALL CLAIMS NOT AETNA. DO NOT CALL AETNA WITH ANY QUESTIONS.

You will receive an e-mail from notifications@alliedbenefit.com. Step (2) Enter all of the required information on the Request Account Information page. Step (4) Follow the instructions on the Notifications e-mail to complete the process
