3 Background 15 primary care asthma performance indicators (PC- API) derived using a modified Delphi approach with a multi- discipline expert panel The PC-APIs are presented in 9 main questions Responses are by point and click or entering a number Feasibility of using this PC-API tool was tested and demonstrated (with 5 sites, 50 prospective patients and 50 retrospective charts) 1. 8 Submit now or Save for later If you choose to SUBMIT then you are done with that patient record, if you have another one you can click the URL link again, another clean page will be provided If you choose to SAVE AND RETURN LATER then you need to save your validation code and you will receive an email to remind you to return later. 9 Quarterly PC-API Report Quarterly report is based on aggregated data, no individual patient level data will be revealed at all We will produce a quarterly report to all participating sites Each site will see their site-specific statistics and the overall statistics If you have less than 6 patient records, no site- specific statistics will be provided, but you will still see overall API statistics 9Asthma Advisory Committee; March 1st, 2013

11 Site physician(s) will need to sign a consent form A URL link will be provided to access the tool Upload each asthma patient’s data by filling out the online PC-API tool using REDCap Patients’ Health Card Number is not required No predetermined number of patient encounters required No deadline or timeline stipulated, you can start/stop anytime A quarterly summary report will be emailed to the site Site-specific data will ONLY be sent to the respective site Summary 11Asthma Advisory Committee; March 1st, 2013
