3. Report Header Title: Patient Reported Outcomes Date: This is the date the report was printed. (This will typically be the date the latest assessment was completed by may also be a reprint) Patient Identifying Information: Depending on site settings: First Name, Last Name, Initials, MRN, and/or HCN Report Footer Boxes to enter ECOG and PPS scores will display at the bottom of the printout.

5. ESAS and PRFS Up to 7 of the latest ESAS and PRFS scores will be displayed in a trended line graph. Date and Channel will appear at the top. K: Kiosk H: Home P: Paper

7. Brief Pain Inventory (BPI) Pain interference from the completed assessment is displayed in a bar graph. Skipped answers are marked by an x (See relationship interference above). If the assessment was not offered the day the report was printed, “ASSESSMENT NOT OFFERED” will appear in place of the bar charts.

8. Brief Pain Inventory (BPI) The Least, Worst and Average pain severity trends from up to 4 of the most recently completed assessments will display in a line graph. If the question was skipped, there will be a break in the graph as seen for Average pain on the survey completed 12/1/15.

9. Brief Pain Inventory (BPI) The relief from Pain Treatments will display in the bottom corner and will be blank if the patient has skipped this question. Scores will range from 0%-100% in increments of 10%.

11. Cancer Fatigue Scale (CFS) Fatigue interference from the completed assessment is displayed in a bar graph. Skipped answers are marked by an x (See relationship interference above). If the assessment was not offered the day the report was printed, “ASSESSMENT NOT OFFERED” will appear in place of the bar charts.

12. Cancer Fatigue Scale (CFS) The Physical, Affective, and Cognitive fatigue severity trends from up to 4 of the most recently completed assessments will display in a line graph. Each fatigue area is colour coded and should also appear If the question was skipped, there will be a break in the graph as with each of the trended graphs the report produces. distinctly in greyscale.

13. Cancer Fatigue Scale (CFS) The Total Scale Score is displayed below the Trends in Fatigue graph and will display the calculate score based on the patient responses for that day. A value of 18 or high will be flagged as high. If questions were skipped in the patient’s response, the score will not appear.

15. Emotional Distress (PHQ-9 and GAD-7) Total scores on the PHQ-9 questionnaire can be categorized as mild, moderate and severe. Categories are: None (0-4), Mild (5-9), Moderate (10-14), Severe (15-27) Total scores on the GAD-7 questionnaire can be categorized as mild, moderate and severe. Categories are: None (0-4), Mild (5-9), Moderate (10-14), Severe (15-21)

16. Emotional Distress (PHQ-9 and GAD-7) For each assessment the 5 of the most recently completed assessments will display in line graphs. The Emotional Distress question responses trend from up to 5 of the most recently completed assessments will display in a line graph. If the question was skipped, there will be a break in the graph.

17. PHQ-9 Suicidal Intent Question If the patient answered positively to the self harm question, they will be prompted with the Suicidal Intent question. If the Patient answers positively or skips the question, the “Assess Suicidal Concerns” label and flag will be displayed. In any other response, the label and flag will not appear.
