Family Caregiver Support Program  DAAS 101 – Long Form – Question # 20: “Does client (care recipient) have significant memory loss or confusion?” - Question # 20 is only for data collection. - “Alzheimer’s disease and related disorders” need to be documented elsewhere in the client/care recipient’s file. Remember that “related disorders” are characterized as both chronic and progressive.

3. Family Caregiver Support Program  FCSP Respite Care Cap - Respite Care is short term and intermittent relief. - Respite Care Cap of $2,500 annually (fiscal year) per client. - Reference: DAAS Administrative Letter No. 12-07 (July 9, 2012).

4. Family Caregiver Support Program  In the FCSP the client is the caregiver. Check to ensure accurate ARMS entries.  For Purchase of Service Agreement – please furnish copy of license issued by the Division of Health Service Regulation.

5. All Funded Services  Division of Aging and Adult Services (DAAS) is currently developing an Electronic Records Policy.  Estimated completion date is the end of this current fiscal year (SFY 15).  Records Retention – based on funding stream.

7. Subcontractors and Subcontracts DAAS Administrative Letter No. 13-15 (August 22, 2013). Effective Date: July 1, 2013. Affects ALL funds administered by the Area Agency on Aging including HCCBG & FCSP. “Ultimately, Community Service Providers are responsible for the services delivered through subcontractors.”

8. Subcontractors and Subcontracts Three basic components: 1.“Subcontractor Performance Evaluation” (SPE) form on each subcontractor due by June 1 of each year to the PTRC AAA. (The revised SPE forms are not yet available.) 2.Exhibit 14A included in the HCCBG service provider budget package and as part of the FCSP RFP. (The revised Exhibit 14A is not yet available.)

9. Subcontractors and Subcontracts 3.Copy of each subcontract for FY 2016 submitted to the PTRC AAA for review by April 30, 2015 or when they are ready for signatures. (This will be a yearly process.)  Review of submitted FY 2015 contracts by the PTRC AAA contracted attorney. - Variety of formats; varied as to sufficiency. - Development of a Regional Template for HCCBG subcontracts & a simplified template for FCSP subcontracts.
