Stroke data – WA Stroke Services Project WA Stroke Model of Care 2012 -develop and maintain a state-wide stroke data collection & quality improvement framework Extensive work was carried out in 2010 by the then WA Stroke Network to develop a data collection framework Consultations have been undertaken with relevant stakeholders, including clinicians involved in previous work to further progress the establishment of a data collection system for stroke care in WA.

WA STROKE DATA COLLECTION WA Stroke Data and Quality Framework Provides the overarching framework for stroke data collection in WA Includes 12 Standardised KPIs - covers acute and rehab stroke care Specifications have also been developed to support the consistent collection of data. It sets out the numerator, denominator, relevant inclusion and exclusion criteria for each KPI. Meet the requirement of the AuSCR data collection as well as significant elements of NSF audit Mapped to Acute Stroke Clinical Care Standards and NSQHS Standards Endorsed by the DG, departmental heads of the tertiary Metro Stroke Units

8. WA STROKE DATA COLLECTION Phase 1: Establishment Phase Step 1 - Establishment of data collection practices and protocols Subacute Community & Aged Care Directorate (SCACD) seed funding to fund a dedicated stroke data coordinator Developing stroke data collection tools and testing applicability Establishing site specific protocols for implementation Formal stroke data collection commenced at SCGH & FSH for all patients admitted from 1 st Jan 2016 – in paper form Will extend to SJOGMPH & RPH Currently trailing stroke data collection tools at regional hospitals SW Bunbury Hospital, Kalgoorlie Hospital, Broome Hospital

9. WA STROKE DATA COLLECTION Step 2 - Development of a stroke database WA Stroke Module has been developed within the Quality of Care Registry (QoCR) - Expect to be in production by end of Feb 2016 SCACD has assumed governance and financial responsibility to support the operation of the stroke module Phase 2: Ongoing sustainability SCACD is committed to assisting sites to collect stroke data through small amount of FTE funding support. The current Premium Payment Program for Stroke has been flagged as a potential source of funding for dedicated stroke data collection and quality support staff The DG has approved the proposal to initiate negotiation with Health Services of this approach

10. WA STROKE DATA COLLECTION Phase 3: Quality Improvement The Stroke Clinical Advisory Group (SCAG) represents a key group to drive quality improvement in stroke care, utilising available stroke data. SCACD will work with SCAG to determine the best methods to compare WA performance to national and international benchmarks. Ongoing work with Patient Safety & Clinical Quality to identify more data based approach to assessing eligibility for stroke premium payment. Phase 4: Linking with national benchmarking initiatives Where possible, the QoCR Stroke Module has utilised the same data definition as that of AuSCR/AuSDaT to ensure compatibility. Data extracts will be developed to facilitate export to AuSCR/AuSDaT once the WA Stroke Data Collection has become has matured.
