Introduction Serving Durham, Wake, Cumberland and Johnston counties Total population of 1.7+ million 186,000 Medicaid lives Alliance is the largest MCO in North Carolina Serving Durham, Wake, Cumberland and Johnston Counties

Key Durham Local Functions Care Coordination provides additional support for special, high-risk populations o MH/SA Care Coordination Supervisor and 15 Care Coordinators o I/DD Care Coordination Supervisor and 10 Care Coordinators Community Relations providers outreach and education to consumers and partners o Community Relations Supervisor and six staff with focus areas throughout the community Serving Durham, Wake, Cumberland and Johnston Counties

Durham Crisis Collaborative Participating organizations from hospitals, outpatient clinics, first responders and community partners Work to find cost-effective strategies to decrease ED costs without compromising care Share data to define who are high utilizers, common characteristics and points of intersect, and to develop targeted interventions Serving Durham, Wake, Cumberland and Johnston Counties

Peer Drop-In Serving Durham, Wake, Cumberland and Johnston Counties Offers a variety of recovery-based programming Provides a welcoming environment and opportunity for social interactions without an appointment Trains and certifies Peer Support Specialists

Pharmacy Programs Patient Assistance Program provided almost $3 million in no-cost name brand psychotropic medications last year 900+ consumers receive reduced-cost medicines per month Connect consumers while in inpatient care to pharmacy programs to ensure continuity of medications Serving Durham, Wake, Cumberland and Johnston Counties
