3 The Camden Coalition of Healthcare Providers (CCHP) is a strategic initiative designed to improve the quality, capacity, and accessibility of the healthcare system for vulnerable populations in the City of Camden. Activities of the CCHP include: -Community Outreach & Health Education -Case Management of High Needs Patients -Health Provider Education -Practice Management Capacity Building -Data Collection and Evaluation Camden HIE

4 The CCHP, under the leadership of Dr. Jeffrey Brenner, created and implemented the Camden Health Information Exchange. This effort is supported by Cooper Hospital, Our Lady of Lourdes Hospital, and Virtua Health. The Camden HIE is one component of a broader set of initiatives to improve health & healthcare in Camden led by the CCHP. The goal of the Camden HIE is to expand access to health information across the City of Camden to: -Decrease Cost -Improve Access -Improve the Quality of Care Camden HIE
