Evaluation Forms n General Evaluation Process –Reasons –Document flow n Evaluation forms –Basic Cadet (in-flight) –Staff

Evaluation Forms - Process n Purpose of evaluation forms n General Evaluation Process –Definition: Ratee, Rater, Additional Rater –Document workflow (how it is passed around) –Mentor’s Input & the Signatures –Dispute handling –What do I do with completed forms? n KEY FACT: Only 1 evaluation is done

Evaluation Forms - Basic n Basic Cadet Evaluation Form –Flt CC is rater, with Flt Sgt + TAC input n How to use checkbox section –Circle appropriate phrases to describe behavior, even if not in column checked n How to use remarks section –Write neatly, in bulleted statements –INPUT+ - be specific, talk about behavior n Mentor’s Input + Signature

Sample Basic Cadet Evaluation

Evaluation Forms - Staff n Cadet Staff Evaluation Form –Who is rater + additional rater? n How to use checkbox section –Circle appropriate phrases to describe behavior, even if not in column checked n How to use remarks section –Write neatly, in bulleted statements –INPUT+ - be specific, talk about behavior n Mentor’s Input + Signature

Sample Staff Evaluation

Counseling Feedback Tracking Form (CFTF) n Purpose of CFTF –Document adverse behavior that leads down the path of dismissal –Documents counseling in cases of dismissal n When to use CFTF –Repeated bad behavior that normal verbal reprimands do not fix, which require raising the issue up the chain of command repeatedly –Serious, first time offenses that don’t quite warrant immediate dismissal, such as abandoning duties or fraternization

Counseling Feedback Tracking Form (CFTF) n When NOT to use CFTF –Late for formation; forgot to salute; etc. CFTF IS NOT A DEMERIT!!! –Minor offenses or errors –Annoying behavior (i.e. you just don’t like them) n COC seeing this form = Level V = BAD (COC is last signature before dismissal) n Senior mentors sign at each step (Each level on the form describes which senior signs) n Blank forms are kept in Admin office

Counseling Feedback Tracking Form (CFTF) n How to use CFTF –Fill in comments about behavior in appropriate level block –Sign it –Person counseled initials, indicating only that they were counseled, not that they necessarily agree –Have senior mentor present and sign –Return form to admin; retrieve from admin if needed again –Separate forms for separate problems n What do I do with it after encampment is over? –Admin office keeps it on file if dismissed –Admin office destroys it if no dismissal

Sample Counseling / Feedback Tracking Form

Inspection Standard n Purpose of NYWIS –Standardize inspection scoring year to year n RESULT: Allows comparison of this year to others –NOT the cadet OI- tells SET how to score and generally what needs be inspected, not where stuff goes n What the SET needs to do (w/CC staff) –Select optional inspections –Develop encampment specific OI’s –Use appropriate forms for standardized scoring –Compare results to prior years + report n All the encampment needs to do is meet standards set by SET

REMEMBER: Don’t Drink and Drill
