SUPERVISOR’S ROLE WHAT IS THE SUPERVISOR’S TASKS WHEN AN EMPLOYEE INJURY IS REPORTED? LET’S CALL OUT THE ITEMS AND CONSIDERATIONS

IMPORTANT ITEMS You are responsible – “the lead” Make sure the employee is OK if medical attention is needed get it Verify the reported hazard and correct it or cordon it off Remove defective equipment from the environment

ACT BASICS Logging In Should automatically log in when using a computer at CDTA except INIT computers When prompted to login User Name: cdta\{username} Password: {CDTA password}

ACT INJURY REPORTS Ensure you are opening the report from the correct location. Fill out as much as you know at the time. As long as you do not submit it you can go back and fill in more. Question change based on type of Accident.

PICTURES: HOW TO Ensure you have set Date, Employee and Location before uploading pictures Problems from INIT computers should be fixed Pictures have to be on the computer not on the camera or phone to upload If uploading multiple does not work try one at a time
