How many incidents have occurred ? Do the figures compute? When, where, to whom and how have they occurred? Why have they occurred? To know why – we must investigate fully.

Is there a set criteria for establishing what should be investigated? How can we investigate all or a large % of incidents? What tools do we need to do this? What resources /staffing would we need? What training do we need? Who should investigate?
