CONCUSSION MANAGEMENT AGREEMENT Progressive Physical Activity Program Step 1: Light aerobic exercise- 5 to 10 minutes on an exercise bike or light jog; no weight lifting, resistance training, or any other exercises. Step 2: Moderate aerobic exercise- 15 to 20 minutes of running at moderate intensity in the gym or on the field without a helmet or other equipment. Step 3: Non-contact training drills in full uniform. May begin weight lifting, resistance training, and other exercises. Step 4: Full contact practice or training. Step 5: Full game play. If symptoms of a concussion re-occur, or if concussion signs and/or behaviors are observed at any time during the return to activity program, the athlete must discontinue all activity and be re-evaluated by their health care provider. UNIFORMS ***SCHOOL ISSUED PROTECTIVE EQUIPMENT WILL BE PROVIDED**** Lockers & Combination Locks ARE issued Game jerseys provided Athletes responsible for clothing cleanliness (NOT MOM) Proper uniform required for each practice & game (No cut off sleeves, double shorts, etc.) Mouthpieces are provided & MUST be attached to the helmet. PLAYERS ARE RESPONSIBLE FOR ALL ISSUED EQUIPMENT

P.R.I.D.E. PROGRAM P.R.I.D.E. stands for Personal Responsibility in Discipline and Education. We believe this program will help the student-athlete stay disciplined in their grades and conduct. If a student is NOT passing their grade check at the third week, they will be given a P.R.I.D.E. folder. This folder will include a tutoring schedule for each class, weekly grade checks, a tutoring log sign-in sheet, and a conduct chart. Each student placed on the P.R.I.D.E. program will be responsible for attending weekly tutoring and getting their grade checks signed.

P.R.I.D.E. PROGRAM If an athlete is not passing their next grade check, they will miss the following week’s game. The student will not be able to return to play until they turn in a passing grade check. When an athlete turns in a failing grade check, they have until the following Tuesday to bring in a note from their teacher, stating they are passing. If a note is turned in by Tuesday, they will regain eligibility to play in the next week’s game. Students will continue to use the P.R.I.D.E. folder until report cards come out. At that time, we will reevaluate the athlete’s grade and identify if they will remain in the P.R.I.D.E. program and on the team.

STUDENT ATHLETE CONDUCT If any teacher or administrator has to report to a coach about an athlete’s conduct three or more times, they will also be placed on the P.R.I.D.E. program. Misconduct can result in limited playing time and/or missing a game.

**** PARENTS WILL SIGN GRADE SLIPS **** WEDNESDAY: Pickup and Have Teachers fill out and sign Bring grade slip home to be signed. THURSDAY: Return SIGNED Grade Slip Signed by parent to the coaches.

PRACTICES ATHLETIC CLASS – Student will not be able to practice until cleared on their physical ATTENDANCE – Required each session EXCUSED ABSENCE – Head Coach must be notified (Note, E-Mail &/or Voice Mail) UNEXCUSED ABSENCE – AFFECTS Athletic Class Grade & Playing time BASIS FOR DISMISSAL FROM THE TEAM.

TRAVEL PLAYERS WILL TRAVEL TO/FROM GAMES WITH COACHES ON SCHOOL BUSES EXCEPTIONS MAY BE MADE IF REQUEST IS SUBMITTED IN WRITING TO THE HEAD COACH AT LEAST 1 DAY PRIOR TO THE GAME GO FLYERS!!! WIN S.T.A.C.!!

PARENT EXPECTATIONS & CONDUCT TRY-OUTS ARE CLOSED TO THE PUBLIC PRACTICE SESSIONS OPEN TO PUBLIC SPECTATORS WILL REMAIN AWAY FROM PLAYING AREA, GAME OR PRACTICE SNACKS/DRINKS ARE NOT ALLOWED ANYTIME DURING GAME OR PRACTICE ONLY COACHES & PLAYER PERSONNEL ALLOWED ON SIDELINES DURING GAMES SHOW RESPECT TO OFFICIALS & OPPONENTS AT ALL TIMES PLAYING TIME & POSITION STATUS WILL RESULT FROM COACH’S EVALUATION DURING PRACTICE
