Health Improvement Plan Since 1995 PH has led this countywide partnership. HIP’s purpose is to facilitate the design and implementation of data driven collaborative community-based disease prevention and health protection strategies that promote the health of everyone in the county. Partners include: health systems and health care providers, community agencies, universities, county residents

Data Driven HIP Survey every 5 years—BRFS plus Morbidity and mortality 2020 Objectives Focus areas: physical activity, healthy eating, tobacco reduction and depression

Searchable, database, online, generates bar charts and geographical/mapped data by race, age, income, health status, gender ; new non-survey data will be available online soon
