5. Health Improvement Plan Since 1995 PH has led this countywide partnership. HIP’s purpose is to facilitate the design and implementation of data driven collaborative community-based disease prevention and health protection strategies that promote the health of everyone in the county. Partners include: health systems and health care providers, community agencies, universities, county residents
