Active Lifestyle Interactivity Active lifestyle divided into 4 stages: Stage 1: Introduction to exercise Safety test: 80% pass mark; all mandatory questions must be correct. Stage 2: Exercise programme & diary Daily 30 min exercise programme over 5 days Stage 3: Progression stage Risk factor quiz and 5 days of 30 mins exercise Stage 4: Maintenance and progression

Selection criteria - Present for CR! No significant anxiety/depression ISWT performance in excess of 250m Computer literate Introduced at time of pre rehabilitation assessment and password given /registered on site with basic details

Challenges Uptake low Limited capacity for conventional rehabilitation Delays in attending rehabilitation course (return to work/loss of engagement) Focus group – lack of timely information / ‘left in limbo’ Lack of medical endorsement for rehabilitation

Aims and objectives To develop an education component to deliver immediately post discharge for patients and relatives (waiting less than 3 weeks to commence rehabilitation). The rehabilitation programme will concentrate on adopting a healthy lifestyle with directed support to assist in behavioural change. The overall objective will be to have early patient engagement in the CR process to improve up take and compliance to CR.

outcomes 12 month data on patient uptake from point of referral to be evaluated to determine effectiveness. Patient feedback via satisfaction survey.

Tasks Complete review of educational materials used. Filming of patient testimonials & consultants endorsements. New venue & risk assessments. New revised invite letters etc. Satisfaction questionnaire. Ongoing review & amendments.

Investing in CR team Myers Briggs 1 day course for all the team. Motivational Interviewing 2 day training course. Solutions based practice 2 day training course.
