Rural Health Network Planning 101 Purpose - To assist in the planning and development of an integrated health care network. Goals: 1.Focus on the value of services and not volume. 2.Create a foundation for infrastructure. 3.Create the opportunity for collaboration between different members of health care delivery system. 4.Address emerging community health needs and challenges.

Eligibility Requirements The lead applicant must meet 3 eligibility elements – 1.Rurality, 2.Non-profit/public designation, and 3.Have a network composed of at least 3 separately- owned organizations.

Project Scope Applicants were asked to identify one focus area that their network planning activities will address: 1.Population Health A.Care Coordination B.Patient Engagement C.Data Analytics/Health Information Technology D.Physician and Health Care Provider-Hospital Alignment 2.Alleviating Loss of Local Services and Access to Care A.Rural Hospital Closure/Conversion B.Rural Hospital Financially At-Risk of Closing C.Enhancing Sustainable Emergency Medical Services D.Telehealth

Project Scope Applicants were asked to describe planning activities that support at least one of the legislative aims listed below: 1.Achieve efficiencies 2.Expand access to, coordinate, and improve the quality of essential health care services 3.Strengthen the rural health care system as a whole

Funding Restrictions 1.Applicants may request funding for a project period of up to one (1) year at no more than $100,000. 2.Funds under this announcement may not be used for the following purposes: o To build or acquire real property o For construction o Provision of direct services
