2 Need Health Care Plan Evaluative Measures Impact

3 Need Health Care Plan must address all needs discussed in the Needs section. In Needs section, select a health indicator that is reflective of the Performance Measure. When applicable, use data from the health indicator in key factor description.

4 Health Care Plan –SAC, NAP Seven Focus Areas: Diabetes Cardiovascular Cancer Prenatal & Perinatal Health Child Health Behavioral Health Oral Health

5 Health Care Plan –SAC, NAP Eight required clinical goals and objectives: Six defined by HRSA Prenatal PM not required if applicant does not provide primary responsibility for patient’s prenatal care services Behavioral and oral health defined by CHC May add up to two additional goals

6 Health Care Plan –SAC, NAP Objectives must be reasonable, measurable, and reflect an anticipated impact upon the specified need or focus area. Objectives must have a timeline Example: Diabetes: increase the % of patients with HbAlc below 9 from 70% to 75% by 2014.

7 Health Care Plan – EMC, etc. Data provided should not be based solely on the current or proposed patients of the organization but rather, on the total stated target population May use 330 HC Plan, if applicable to target population of grant application. Be sure that Need is defined for the target population addressed in the application Be sure HC Plan is reflective of the needs addressed in Need Section

8 Health Care Plan Healthy People 2010/2020 – Get familiar with the Healthy People 2020 goals and objectives Identify in the comment section the HP 2020 goals that are addressed by each goal Health Disparities – Present 2 – 3 needs with disparity comparisons. Some goals may address both HP 2020 and health disparity. Program Narrative MUST be consistent with the HC Plan

9 Evaluative Measures Summary of the key factors that the applicant anticipates contributing to or restricting progress on the state Health Care Plan goals Discuss broadly. Do not need to provide detail at an “action step” level. Include reference to any other evaluative measures, i.e. the Quality Improvement Plan.

10 Impact Can reference Health Care Plan in Section 4.1.b. Addressing the target populations’ identified health care needs.

11 Data Collection Source of performance measure data, method of collection and analysis (e.g. EHR, disease registries, chart audits/sampling) must be noted Data must be valid and reliable Data should be derived from currently established management information systems, where possible

12 Data Collection If baselines are not yet available – applicant states when data will be available Additional measures chosen by the applicant should also define the numerator and denominator that will be used to determine the level of progress on the goal

13 Remember: The Program Narrative must be consistent with the Health Care Plan. Information provided on need should serve as the basis for, and align with, the proposed goals and activities described in the health care plan. Data provided should not be based solely on the current organization’s patient population but on the total stated target population.

14 Remember: The Health Care Plan must address all needs described in the Needs Assessment. The objectives must be measurable and time lined. The required goals do not restrict you from addressing other needs in the community. The HC Plan is not a complete plan of care or a set clinical protocol. The Quality Assurance Program can address other areas of care.

15 Remember: The CHC will be evaluated on meeting the needs identified in the needs assessment and included in the Health Care Plan. All targets in the goals will not be met in the first year, but rather by the end of the project period. Be Consistent throughout the application with data. Keep it simple. Address all requirements but don’t write just for the sake of writing!
