1 West Baltimore CARE Health Careers Scholarship Program June 2016 Overview

2 What is the WB CARE Health Careers Scholarship Program? Goal is to help increase the number of health professionals who live and work in West Baltimore Provides financial scholarships that cover some or all of the tuition and education-related expenses for health career programs Scholarship recipients are selected via a competitive process which includes a submitted application, recommendation letters and a face-to-face interview In return, individuals that receive the scholarship commit to work as a health care professional in West Baltimore for two years 2

3 Who is Eligible for a Scholarship? Live in West Baltimore Zip Code of 21216, 21217, 21223 or 21229; Are over 18 years old or have a GED or High School Diploma; Commit to working in West Baltimore for two years; and, Have applied, been admitted to or are enrolled in an eligible health career program. 3 We are unable to provide scholarships to all eligible applicants due to limited scholarship dollars.

If your health program is not listed, contact our office (410-368-2192) to see if it is covered.

5 Covered Tuition Books Education-related expenses Supplies and Equipment Not Covered Computers Tablets Transportation Meals Living Expenses 5 How Much are the Scholarships and What is Covered? You may be awarded up to $2,500 based on the cost of your program. The scholarship award will be paid directly to the school once invoiced. We are unable to provide scholarships up to $8,000 this year due to scholarship demand, limited funding and the shortened time period within which the scholarship award must be used.

6 When Must I Use My Scholarship Award? Your must use the scholarship award no later than January 31, 2017. We must receive the final invoice from your school no later than February 28, 2017. If you do not meet these dates, your scholarship will be terminated; and, you will be responsible for any payments to your school for the award amount. 6 **You are responsible for following up with your school to ensure that they submit your invoice for payment prior to February 28, 2017**

8 How Do I Apply? The Health Careers Scholarship is competitive. You must submit a completed application package to be considered. If your application is missing any information, you will be immediately disqualified. We will be accepting Scholarship applications beginning June 13,2016 through June 30,2016. Please go to our website to obtain an application - www.healthywestbaltimore.org. www.healthywestbaltimore.org All applications must be received no later than June 30, 2016 via e-mail. 9 How Do We Select Scholarship Recipients? Selection Factors  Eligibility. Applicants must meet all program eligibility requirements to be considered.  Completeness of application. Applicants are encouraged to submit a complete and responsive application.  Commitment to a career in health care and working in West Baltimore as demonstrated by your responses to the essay questions, recommendation letters, and interview questions.  Please note that preference will be given to first- time applicants. 9

10 What Is a Complete Application Package? 10 You Must Include: Signed and completed Application Form Two signed Recommendation Letters  Letter must include: –Your relationship to the reference (cannot be family member), –Length of time you have known each other, –Why you are a good candidate for this scholarship, and, –Reference’s full name, address, phone number, e-mail address and signature. Proof of residence in one of the 4 Zip Codes (21216, 21217, 21223, 21229) State-issued identification such as your driver’s license or ID card, Recent utility or other bill, or Mortgage statement, rental or lease agreement Proof that you have applied to, were accepted or are enrolled in a program Letter from Program Confirming Receipt of Your Application Acceptance Letter from Program Invoice from Program If your previously received a scholarship from us, you will be required to provide a copy of your current transcript with grades.

11 1)You will receive an e-mail confirming receipt of your application package within three to five work days. 2)You will be scheduled for an interview if you are eligible and your application is complete and demonstrates your commitment to a career in health care working in West Baltimore. 3)After the interview, you will receive an e-mail telling you whether you have been awarded a scholarship by the Scholarship Award Date. 4)If awarded a scholarship, you will be required to sign a Scholarship Agreement and participate in a two day Career Readiness Training. Failure to sign the Scholarship Agreement and attend the Career Readiness Training will result in the termination of your scholarship award. 11 What Happens After I Send in My Application?
