2 SCOE’s Adult Re-Entry Services 2 Sacramento Community Based Coalition Opened in 2007 - Parole Clients, 3500+ Woodland Day Reporting Center Opened in 2013 - Parole & Probation Clients, 500+ In & Out of Custody Programming West Sacramento Day Reporting Center Opened in June 2014 - Parole & Probation Clients, 65

3 PREP Program Model 3 Wrap-Around Life Skills; Education; Treatment; Employment; Referrals Individualized Individualized case service plan; client centered Phase-Based Sequential program; build foundation / relationship at entry Partnerships Create environment where partners can function effectively with us – “One Stop” Cutting Edge Current and Relevant Evidence Based Practices; High “Dosage”; 80% attendance; risk level classification; morning and afternoon classes offered

4 PREP Program 4 Began in May, 2015 with In Custody programming Expanded to the PREP Center in August, 2015 Partnership between Probation Department, Sheriff’s Department and SCOE Class Offerings in Cognitive Behavioral Education, Pre- Employment, Money Management, Drug / Alcohol Education Current Enrollment & Demographics

5 5 In Custody Program Refer / Enroll 2-6 Months Prior to Release Schedule and Curriculum 5 days a week, 3 hours a day Evidence-based Texts Individual Service Delivery Assessment; Referrals; Transition / Continuum of Care Re-Entry Management Identification; Needs / Services; Transition

6 Out of Custody Program Individualized Case Service Planning Based on Assessments, Supervision Conditions, Client Interests Phased-based Programming Program Intake & Orientation; Risk / Needs Assessments; Individualized Service Plan (“Passport to Success”) Development Foundational & Individual Classes; Workshops; Referrals Personal Life Plan; Continuing Education and/or Employment Services and Community Bus passes; Shuttle; Food; Clothes & Suits Closet; Computer Lab Culture of Success & Feedback Loop (Client of the month, satisfaction surveys) 6
