7. IV. Fatality Process Victim got to work at around 09:00 on Jan O, OOOO by a commuting bus. Victim performing the arranging task of the washed and dried laundry at around 09:00 ~ 11:00. At around 11:00, victim had fed the laundry to the ironing machine with co-worker (as in pair). Victim died from neck being pressed by the scarf which was caught in the gap(7mm) between the feeding belts and wound on the rotating pulley while the victim with scarf worn was feeding the laundry to the ironing machine at around 13:30. Employer must train the workers how to work and provide protection system after identifying the work hazards when the workers might be exposed to the hazards before they operate the machine. Victim was the intellectual and developmental disabled (class 2) who needed special care. Move the Operation Panel and Install the Emergency Stop device –Move the Operation Panel of the Ironing Machine to the place where the worker can reach in an emergency. –Install an Emergency Stop device or switch with which the worker can stop the machine immediately in an emergency.
