New Sites Subawards $7k (Year 1), $10k (Years 2 & 3) YEAR 1 Establish protocols for formal mentoring program Develop a start-up plan with mentor recruitment strategy Participate in all mentoring trainings and technical assistance Recruit, screen, and train 7 mentors in preparation of launch Launch group mentoring by 9/30/2014 3 - 7 matches by 12/31/2014 YEARS 2 & 3 Maintain 3- 7 mentoring relationships that started in prior year Recruit, screen, train additional mentors for next round 16
