Filling out the Student History Worksheet

Initial first day of school in U.S. Schools The date that student was first identified as Limited English Proficient The number of school years of enrollment in a U.S. school starts with grade 1, or the first school year thereafter if the first school year of enrollment in the U.S. is later than grade 1. Kindergarten and prekindergarten do not count because school attendance is not mandatory until grade 1. Exceptions are not made for partial school years of enrollment. The value for first grade ELLs should never be higher than “1st school year” unless the student was retained in grade 1.

Here, Ima Sample enrolled for part of the year at Cavazos. Then withdrew and attended Henderson where Mrs. Mitchell filled out her TELPAS information. It is blank for Cavazos because Ima was not there for TELPAS.

For the 2011-2012 school year, Ima remained at Henderson all year where she received an Advanced rating on Reading TELPAS and she took the general STAAR.

Most of the information for the current school year is blank because TELPAS and STAAR will be this Spring. We also do not know if the student is going to remain all year at Henderson.

The suggested TEA form and information was found at the link below. http://www.tea.state.tx.us/student.assessment/ell/lpac/
