2 Overview Underground coal mine (room and pillar) (2013: approximately 805,720 h worked and 322 employees) 47-year-old general laborer Victim was driving a battery-powered, two-man personnel carrier which overturned crushing him Victim was traveling out of the mine – shift started at 3:00 a.m. and accident happened at 1:07 p.m. 2

5 Findings Victim had 15 years of mining experience, with 11 at this mine. Task training was not received Accident happened on a 21-feet wide main haulage road in good condition with no mine- bottom irregularities Loose material from rib sloughage was present Victim was travelling outby and uphill Toxicology report indicated the presence of Butalbital (1.2 mg/l) 5

6 Findings Butalbital is an addictive barbiturate with an intermediate duration of action. Most commonly found on Fioricet, a prescription medicine used for pain, headache and migraines Fioricet’s formulation includes 50 mg of butalbital, 40 mg of caffeine and 300 or 325 mg of acetaminophen Its side effects include drowsiness, dizziness, drunk feeling, feeling anxious or restless 6

7 Findings Victim and another employee were leaving the section when the mine manager asked them to take a diesel-powered personal carrier out of the mine. Victim went ahead driving the battery- powered carrier with colleague behind, on the diesel-powered carrier Colleague stated that he saw the carrier swerve, hit the rib and overturn. Evidence contradicts him, there were no visible tire marks or disruption of the windrow along the rib and a straight line skid mark was found 7

8 Findings An anonymous complaint was filed with MSHA on the day of the accident stating that battery-powered carriers were being pushed by diesel-powered carriers. During investigation, three of the four miners interviewed stated that they have seen this happen before Fresh scrape marks were found on the rear bumper of the battery-powered carrier matching some of the paint markings found on the bumper of the diesel-powered carrier No mechanical problems were found with the vehicle that would have contributed to the accident Scene of the accident was altered in order to allow inby vehicles to advance to the cage. The movement of the battery- powered carrier was not warranted by any emergency situation 8

10 Root causes Mine management did not have adequate programs, policies, and procedures in place to ensure safe travel of self-propelled vehicles on outby travelways of the mine. Miners were not appropriately trained on the policies and procedures 10

11 Conclusion The accident occurred because mine management did not have effective policies, programs, and procedures for safe travel of self- propelled personnel carriers on outby travelways of the mine. Evidence indicates that a diesel- powered vehicle pushed the personnel carrier being operated by the victim, which caused the personnel carrier to overturn on top of the victim 11

12 Best practices Operate all powered haulage, along with trailers and sleds, at speeds consistent with conditions and the equipment used. Control equipment so that it can be stopped within the limits of visibility. Maintain off-track haulage roadways from bottom irregularities, debris, and wet or muddy conditions that affect the control of the equipment. Sound audible warnings when making turns, reversing directions, approaching ventilation curtains, and any time the operator’s visibility is obstructed. Ensure the sound level of audible warnings is significantly higher than that of ambient noise. Maintain mechanical steering and control devices to provide positive control at all times. Provide all self-propelled rubber-tired haulage equipment with well- maintained brakes, lights, and warning devices. 12
