Zufall Health Center – A Federally Qualified Health Center serving Morris, Warren, Sussex, Hunterdon, Somerset and Essex Established in 1990 in a church basement in Dover by Dr. Zufall and volunteer physicians FQHC since 2004; providing entire range of primary medical, dental and enabling services Have 5 sites, 3 Dental Facilities a mobile medical van, and a Wellness Center Serving uninsured, underinsured, homeless, residents of public housing, farm workers, veterans NCQA Accredited Patient Centered Medical Home Bilingual staff 2

Zufall Health 2015 Statistics Served almost 30,000 patients in more than 95,000 visits 91% of patients were below 200% of Federal Poverty Level 50% of patients were uninsured 27% of children 0-17 years old have no insurance 50% of seniors do not have Medicare 66% of patients were Latino

Expanding to Serve the Need in Our Local Communities Opened Hackettstown site in 2012 – to serve residents of Public Housing, MSFWs and general population Opened Flemington site in 2013 – Dental only Opened Somerville site in 2014 West Orange NAP – Opened June 2014 Morristown Wellness Center – Opened July 2014 Newton (Sussex County) - coming April 2016

State Program Application acceptance on rolling basis. This program offers $120,000 in loan redemption over 4 years and requires a minimum of 32 hours/week for the first two years. After 2 years, participants can drop back to part time. Only General Dentists and Pediatric Dentists can apply for loan redemption through the state. FOR MORE INFORMATION & APPLICATION REQUEST: 973-972-4605 Loan Redemption Program Federal Loan: National Health Service Corps The advantage of this program is that it offers full time (40 hours) and part time (20 hours) options. This program is based on a HPSA score and priority is given to General Dentists and Pediatric Dentists in sites with a score of 14 or higher. AmeriCorps Volunteers What is AMERICORPS? Americorps engages Americans in intensive service each year at nonprofits, schools, public agencies, and community and faith-based groups across the country. Americorps programs do more than move communities forward; they serve their members by creating jobs and providing pathways to opportunity for young people entering workforce. AmeriCorps places thousands of young adults into intensive service positions where they learn valuable work skills, earn money for education, and develop an appreciation for citizenship.
