CNAP 1-877-540-6565 What is CNAP? A network of over 30 Community Support Services agencies - not- for-profit organizations- working together to serve seniors in communities across Toronto. Seniors, their families and care providers are encouraged to call the CNAP toll-free phone number when they don’t know where to turn for help at home. 3

CNAP 1-877-540-6565 Community Support Services (CSS): Free, Fee-Based or Subsidized 4 CSS – community support services are publically funded by the Ministry of Health and Long Term care through the Local Health Integrated Network (LHIN) CSS services are either free, subsidized, or *fee-based for clients No OHIP card required * eg Home Help, Respite, Transportation, Meals on Wheels, Adult Day Programs

CNAP 1-877-540-6565 Call CNAP Seniors, their families, or care providers call 1-877-540- 6565 to connect with CNAP. They are greeted by a professional Intake and Referral Social Worker who takes the time to understand each senior’s needs and connects them to the right service provider in the local community. The toll-free number is staffed Monday to Friday, 9am to 5pm. Voicemail will be returned next business day. 9

CNAP 1-877-540-6565 Explain Your Needs Caller explains his/her needs, and the social worker will suggest services that match the need of the clients. To ensure privacy and confidentiality is maintained, the social worker obtains consent from the client / substitute decision-maker/ caregiver, etc. Demographic data (i.e. date of birth, address and postal code, alternate contact), living situation and health condition are necessary for the referral. 10

CNAP 1-877-540-6565 Access to Services Client is matched to a particular agency based on the following criteria: o the services required by the client o the catchment area in which the client resides o factors such as client’s language, preference of agency, Frail, At Risk & Marginalized (FARM) status, are also considered in the matching process Client is informed of the agency that he/she will be referred to (target agency) and that the agency will be contacting them within two business days to arrange for service(s). 11

CNAP 1-877-540-6565 Access to Services (Con`d) After speaking with the client, the CNAP Social Worker will: –Contact the Intake Worker at the target agency –Confirm that the client can be serviced by the agency –Confirm client lives within the catchment area –The CNAP referral is then sent to the target agency who then confirms receiving the information and then contacts the client to arrange services. 12

CNAP 1-877-540-6565 Connection to non-CNAP Agencies CNAP Social Workers make every attempt to connect seniors to services even outside of the CNAP Network of agencies such as: Community Care Access Centre Organizations and Networks within and outside of the Toronto Central LHIN catchment. Community agencies that provide specialized services to Seniors such as CNIB, The Canadian Hearing Society, Alzheimer Society, LOFT Community Services, etc. 13

CNAP 1-877-540-6565 Remember If a client is already receiving services from a service provide, they are encouraged to contact them directly. If you don`t know which agency can help you, clients, caregivers and healthcare professionals should call the CNAP line where a connection will be provided. CNAP provides a connection to services. It is not for emergencies or counseling support. The CNAP line is staffed by English & French speaking Social Workers. 14
