1 2015-16 Student Performance Evaluations Program Coordinator Instructions

2 ASSIST Student Performance Review Instructions This year, our student performance evaluations will be conducted online in a process very similar to the online reflections. They are to be completed and submitted to our office by June 15. This performance review will collect information from ◦the student's self-assessment and a performance feedback from their ◦work supervisor, ◦facilities management, ◦and the senior mentors' they serve. We have designed the workflow so that you do not have to enter all the data at one time. You will have the ability to go back and add evaluations/feedback before finalizing the student’s review.

3 Step 1 You will direct your students to www.southern.edu/assist and click on the link found on the lower portion of the front page entitled: ASSIST Student Performance Review.

4 Step 2 Students will select “Student” and complete their self-assessment. For Q7, instruct students to select 2015-16 For Q8, instruct students to select Spring Semester

5 Step 2, continued Once the student has answered Q27, the student will skip Coordinator Password and Question 28. Then, they will click Continue at the end of that page and on the next page, click Submit.

6 Step 3 You will receive an email indicating the student has completed their self-assessment. When you are ready to provide feedback to the student about their performance, click on the link provided. If you don’t have all the performance reviews from facilities managers or senior mentors, keep this email to access and input data as you receive it. No other emails will be sent until the evaluation is final. You will select Program Coordinator for the “Person submitting this performance review” and use the same password as you use to approve student reflections.

7 Step 4 Do you have all your information? Yes, I have collected the performance reviews from the facilities manager and the senior mentor(s) and I am ready to give my feedback of the student’s performance. Select “Completed” for Q28. This question appears after the student self-assessment. No, I am still missing some information but I want to go ahead and enter my feedback of the student’s performance and the feedback from the social activities director at the facility. I am still waiting for the feedback from the senior mentor(s) my senior mentor liaison is collecting. Select “Started” for Q28. You will always select “Started” until all data is complete.

8 Step 4, continued Just a reminder that at any time you are working in the student’s performance review, you will need to always click “Submit” to save your work. That is why it is so important that you accurately choose either “Started” or “Completed” in Q28.

9 Clarification on Performance Feedback It is at this point, you or another work supervisor will provide feedback for the student. The person that observes the student most often should provide the feedback. There is other feedback you will need to provide. ◦You will collect feedback from the facility administrator or other facility manager that is familiar with the student’s performance. Examples could be the social activities director, director of nursing, or family member. And, in some settings – such as individual homes, there may not be anyone to provide this feedback. If there is no one to provide this feedback, please state in Q40 why it is not available. ◦You or your senior mentor liaison will collect feedback from at least one (1) senior mentor, but no more than three (3). ◦I have attached those forms to the website and via email so that the information can be gathered onsite. The information from the completed paper forms will then need to be entered into the student’s performance review.

10 Step 5 – Completed review Once you have completed the performance review, you will receive an email stating that the performance evaluation for your student has been completed. You will then see a PDF of the complete performance review included in the email. Please share this complete review with your student to either congratulate or coach for improvement.
