Enrollment Application For 16 and 17 year olds, who left K12 schooling without a diploma Used to request permission to: enroll in the program take the GED ® Test Underage Youth Application

Enrollment Steps for Students 1) Fill-out the first page a) Parent/Guardian signature (some exceptions) 2) Acquire a withdrawal form or letter from last school (some exceptions) 3) Present photo ID confirming age Underage Youth Application

Parent/Guardian Permission Parent/Guardian completes form in the presence of a witness printed name signature date Underage Youth Application

Parent/Guardian Exemptions* Married Emancipated Court Ordered/Adjudicated Enrolled in a Special Program *with documentation Underage Youth Application

Withdrawal Form or Letter Withdrawal form from last school attended Letter from the superintendent verifying the applicant is no longer enrolled or was never enrolled Underage Youth Application

GED ® Testing Approval Steps Attend class for at least 40 hours Score at least 9.0 on the D or A level TABE in Reading, Math, and Language Schedule a time with his/her teacher to create a profile on MyGED™ at www.ged.com and take the GED Ready™ practice test and score at least a 150 on two of the four subtests www.ged.com Underage Youth Application Another option is to take Aztec’s GED® Practice Tests (if available) and score 60% or higher in the “Ready” zone on two of the four subtests. Agree to maintain enrollment until they successfully complete GED ® testing

GED ® Testing Teacher recommends the student and program administrator gives final approval Program submits Request for GED Testing Approval to Georgia GED Testing Program Approval takes about 2 business days and the student will be notified by email Underage Youth Application
