HEALTH CO-MANAGEMENT SECRETARIAT WWW.HCOM.CA HSIF Proposals – Update Collective sessions to draft HSIF proposals Two sessions: September 13-14, 2011 October 4, 2011 One large room, 4 groups, 4 writers, 4 facilitators, up to 90 participants Broad participation from First Nations communities / tribal councils / Treaty organizations, Alberta Health Services, Alberta Health & Wellness and FNIH Draft proposals circulated before and after the October session Proposals reviewed and approved by the HSIF Working Group, the Governance & Capital Subcommittee and Co-Management

HEALTH CO-MANAGEMENT SECRETARIAT WWW.HCOM.CA HSIF Projects – Priorities To develop and implement First Nations Continuing Care Model for a continuum of care through active participation of all stakeholders based on cultural best practices while integrating with identified and sustainable existing resources Starting with a progressive implementation of the model and then fully implement across Alberta Development of a First Nation Continuing Care Model CA Host: Treaty 7 Management Corp. First Nations Mental Health and Addictions Services Integration Strategies CA Host: Bigstone Improved continuum of care for First Nations people suffering from substance abuse and addiction through integration of mental health and addictions services Improved crisis response services for First Nations communities Standardize and improve the discharge planning process for First Nations people upon release from hospitals and mental health facilities Establish more effective prevention strategies through improving service coordination and information sharing

HEALTH CO-MANAGEMENT SECRETARIAT WWW.HCOM.CA HSIF Projects – Priorities Basic primary care services (e.g. visits to the community by a general practitioner or a nurse practitioner referrals to specialists using telehealth) Chronic disease prevention and management Cancer screening: Breast, cervical, and prostate and referrals Information sharing agreements to enable the sharing of health information for the coordination and continuity of primary care Provincial Support for Integrated Primary Care on-Reserve CA Host: Kee Tas Kee Now Tribal Council Exploring Partnership in First Nations Health Governance CA Host: YTC & Maskwacis Health Services Phased approach to the exploration of a health partnership Activities will create and support the environment necessary politically and administratively for First Nations to develop and implement health priorities for seamless health service delivery through a revitalized relationship with federal and provincial governments and those delivering health services Each partner respects the important contributions of each other and affirms their commitment to establishing mutual objectives in First Nations health
