Implementation and Settlement: Implementation 2 StepsLead Org Activity (LO) Endorsing Org (EO) Activity Ministry Activity Lead Organization Consultants will construct project LO will ensure project remains on schedule and within approved scope and cost No ActionMonitor progress and fund construction based on projected and actual progress within each fiscal year Review requests for change in scope Funding of construction will be managed by the Branch managing the project Each branch will be responsible for managing their portfolio of projects within their annual notional allocation Any change orders that constitute a change to approved scope will require ministry approval and amendments to and re-approval of the Business Case, Cost Estimate and Agreements

Implementation and Settlement: Settlement 3 StepsLead Org Activity (LO) Endorsing Org (EO) Activity Ministry Activity Lead Organization proceeds to settlement LO will fulfill information requirements as defined by the ministry No ActionAdministrate the settlement process Checkpoint IX: Finalize and Approve the settlement (payment or recovery) Settlement of the project will be managed by the Branch managing the project Cost share calculations per eligibility will be defined by the Community Health Service Provider Cost Share Guide Cost share proportion of cost will be defined by the approved Business Case and Agreement Where additional funding is required, additional funding requests/approval packages will be the responsibility of the branch managing the project

Roles: LHINs in Stage 1: Application Support the consistent application of the CHCP policy and Operational Framework Be the first point of contact for Applications from HSPs for capital projects Provide Endorsement for the Application Confirm any own funded/partner funded space can be financially managed by the HSP/partners 6
