11 Exhibit B Bid - Publicly Opened One Award to the lowest responsive/responsible bidder, that is most advantageous to The Authority, price and other factors considered. 13 Exhibit D This solicitation contains both: Definite Quantity/Definite Delivery and Indefinite Quantity/Indefinite Delivery Performance Period: Contract is for three (3) years Multiple locations Reports Employee Attire – contractor must provide uniforms or attire that identifies the company

15 Insurance Requirements General Liability - $1,000,000 per occurrence including Products and Completed Operations coverage. DART needs to be listed as an additional insured. Automobile Liability - $1,000,000 per occurrence. DART needs to be listed as an additional insured. Workers Compensation – Statutory Benefits and $500,000 Employers Liability. Employer Indemnity type policies will NOT be accepted. Required from Prime & Subcontractors working on site. Waiver of subrogation in favor of DART. Only the successful offeror will be required to submit evidence of insurance.

18 Exhibit H Statement of Work Work Requirements, the Contractor shall provide all labor, supervision, materials, supplies, transportation. Light Rail Safety class is required for work at certain locations, example Dallas Streetcar Stations Quality Assurance Plan Contractor Inspection Report

19 Exhibit H Statement of Work MSDS’s for all materials used Master List- Facility locations and amenities Frequency of Cleaning and Performance Standards.

20 Exhibit L Complete the entire form Requests references and release of financial information to determine offeror’s responsibility, make sure email and phone for contact person are current. Submit form with your bid package

21 Exhibit V Employee Information Report Complete both pages and submit with package

22 Question/Answer All questions must be submitted in writing Written response may be provided as Amendment to solicitation All questions must be submitted through DART supplier portal, even if asked and answered during this meeting
