1. Unsafe Acts
You do not have to be a rocket scientist to determine if a workplace is a safe or not. Sometimes, the answer is very obvious. Of course, we should all know by now, common sense is not that common. Take a look at the pictures below, you will be amazed how people can do such dangerous acts.

2. Unsafe Acts
No respiratory protection at all for a coal miner like him.  He is likely to develop "dark lungs" disease very soon.

As the holiday season approaches, please think about those workers who put themselves in dangerous situations to make a living.   This worker is carrying at least a minimum of 40 kg of coal.  He brings it up to the ground level from a narrow shaft underground, walks a kilometer and drops it off at a weight station.  The more he carries, the more he gets paid.  How much he gets for carrying 40 kg of coal?  Around  $0.20 a trip.  Most likely, he will develop "dark lungs" disease soon due to his exposure to coal dusts.  

4. Unsafe Acts
The wall was being cut.  Proper lockout/tagout procedure was not implemented to ensure wall outlets were indeed deenergized. 
Someone could have flipped the circuit breaker back on.  

5. Unsafe Acts
As you can see, some individuals are sitting on some tiny branches.  Those branches may break anytime.  

6. Unsafe Acts
One slightest misstep will lead to a fall down to the river below.  No safety net, no plank to walk on.

Electricity and water do not get along well.  An electrocution is waiting to happen.  He should be using a cordless drill instead.

Whoever picks this exit as emergency exit really means emergency exit.

With his truck supported by two logs, he is welding near the gas tank.  The truck can fall anytime.  Meanwhile, the gas tank may explode because of the welding.

This worker really wanted to get to the top to work on the street lamp.  However, he was on a very unstable "home-made" ladder.  He could have fallen to death easily.

If the ladder slipped a little bit, these two workers would have dropped to death in a moment.

A sedan is designed to carry passengers and a little bit of cargo.

Either the gentleman in shorts is too careless, or the men in level B suit are too careful.

Ouch!!! Where did the fingers go?  To prevent jamming a bench grinder, the gap between the tool rest and the grinding wheel should be as close as possible.  OSHA regulation allows up to 1/8 inch.  As you know, when the grinding wheel wears out, it gets smaller, and the gap widens.  Therefore, it is necessary to adjust the tool rest once in a while.

15. Unsafe Acts
Once you put a lock on a switch as part of the lockout/tagout procedure, you also take the key with you.  It is to ensure that nobody can remove the lock and turn on the switch.  By leaving the key in the lock, the whole idea of lockout/tagout is therefore defected.

Believe it or not.  A full size container was knocked over by strong wind, and landed directly on a truck below.  Luckily, the truck driver was able to get out in time.

Obviously, these employees all do not have any kinds of fall protection.  In addition, they are likely to be more than 6 feet above the ground level.

Install an air conditioner.  It is hard work.  One of the workers may drop and hit the ground below head-on.

Note the man in the middle.  He is using a piece of foam as his hard hat.

This worker can fall into the ocean at any time.  Plus he is not wearing any personal floating device (aka life preserver).

He is using a metal grinder.  Note he is wearing a saran wrap as his face shield.  Once the wrap melts on his face, the wrap will likely stick on the face for a long time.  

24. Unsafe Acts
His welding mask is a piece of newspaper.  As you know, a single spark from welding operation can ignite flammable substances such as newspaper.
