Female Entrenched Rough Sleeper Project (FERSP) Pan-London 1 Community Mental Health Nurse Cohort of 45 older wandering women Partnership working with homeless agencies Negotiating with mainstream health, housing and social care services to achieve positive outcomes

Joint Assessment Service (JAS) Westminster Only Uni-disciplinary - Social Workers Assessments of vulnerability on behalf of HOS for people who present as ‘priority needed’ due to a mental illness Link applicants to appropriate services (mental health, floating support etc) Provide housing support until the person is settled in permanent accommodation

Homelessness Prevention Initiative (HPI) Kensington, Chelsea & Westminster Mental Health Hospitals Only Working with homeless people and those with tenancies at risk Early identification within 24hrs of admission Complete Health, Housing & Social Care Needs Assessments and Housing Plans MDT including 4 Peer Support Workers Referral rights to step-down beds and placements

JHT Outreach Westminster Only Multi-disciplinary Team Delivering on Health, Housing and Social Care Assessment, Case Management & Treatment Partnership working with homeless agencies Outreach to the street and day centres Time-limited Intensive Case Management Referral rights to crisis beds and longer term placements

How to make a referral? JAS – Only HOS case-workers can refer. JAS check CHAIN so will contact you if the person is known to you HPI – Every homeless person/person with a tenancy at risk is identified by HPI within 24hrs of admission, so you don’t need to make the referral. HPI check CHAIN and will contact you for information and to joint work

How to make a referral? FERSP – will accept referrals for women over 45-years-old who are known in more that two boroughs and don’t have a primary alcohol or substance use problem Call or e-mail JHT Duty and ask for Dagnija O’Connell

How to make a referral? JHT Outreach – will accept referrals from anyone for anyone who is a CHAIN verified rough sleeper in Westminster, with a suspected mental illness and is unable or unwilling to access mainstream mental health services. Call or e-mail JHT Duty to make a referral If it’s urgent call 999 or the Single Point of Assess (SPA) on 0800 0234 650
