3 Southern Health NHS FT: providing mental health, learning disability, community and social care services 8,000 members of staff Staff working in patients homes and from 200 main sites including community hospitals, health centres, and inpatient units Providing care for 250,000 people each year 1.5 million community contacts 280,000 outpatient appointments 730 beds in 23 sites

14 Mental Health Services A simplified Adult Mental Health pathway that supports recovery, formed of Community Mental Health Teams and Acute Care Teams A range of forensic services some of which are highly specialist, eg Bluebird House, one of only two units in England caring for young women with the most complex mental health needs Referral Recovery College Community Mental Health Teams Access & Assessment Community Treatment Discharge Acute Mental Health Teams

15 Mental Health Services Strengths Recovery oriented practice National recognition of excellence – following significant redesign Partnership with Hampshire County Council, plan to full integration Challenges Recruitment & use of bank and agency staff Use of out of area beds – with a range of plans in place to reduce this Estate issues that we are working to address – including Ravenswood House

16 Integrated Community Services Integrated leadership of community based physical and older people’s mental health teams Building block is Integrated Community Team based around general practices Working to join up assessment and caseload management with general practice and social care Inpatients on 10 sites

20 Children’s Services Strengths Hampshire wide Universal and enhanced ‘Healthy Child Programme’ provided by team of health visitors and school nurses Family Nurse Partnership teams focussed on young parents requiring extra support for the first 2 years Delivering Call to Action Key focus on safeguarding Challenges Future integrated models of care
