Activity in 2010/11 progress behind trajectory for 80% roll out ≈2.4 million offers required for 80% roll out ≈1.4 million NHS Health Checks actually offered by end of Q4 activity picked up in final quarter Q4 shows significant improvement →uncertainties lifted 59% take up rate overall (72%- 40% between SHAs)

Plans for 2011/12 NHS Reforms - business as usual for NHS Health Checks PCT plans should enable 90% of a fifth (18%) of their eligible cohort to be offered a NHS Health Check Most of the country aiming for 18% Data will be published

Dataset Significant developments →Implementation phase March – Information Standards Board (ISB) approved at full standard stage June – all conditions removed and Information Standards Notice (ISN) to be issued this week An ISN is a formal notice of an information standard approved by the ISB, and the need to comply

Dataset Data will be extracted by GP software providers and flowed to Information Centre October – aiming for a first test extraction of limited data Early 2012 – full extraction of data to start quarterly PCTs don’t need to extract data Do need to ensure GPs are recording the data on patient records / programme compiling with collection rules. Basic data will be played back to PCTs

Exclusion READ codes Now dataset finalised can provide READ codes To assist with excluding people from the programme Clinical coder has reviewed codes Initial list Expert group will review more thoroughly

Interim I.T. solution Interim solution featured at last Learning Network DH funding CfH to roll out to those interested 16 PCTs installing software, with others interested Trevor Anders, CfH, present to talk through the solution with any one interested in exploring further Information sheet provided in your delegate packs Work continuing on the full solution

Management of high risk people At the last Learning Network, undertook to provide best practice guidance for management of people at high risk Work has been underway NHS I to share what they have learnt Mel Varvel / Richard Healicon running a dedicated workshop

Modernisation of health and social care “Healthy lives, Healthy People” sets out Government’s long-term vision for the future of public health in England. Confirmed the continuation of the programme Proposed –as an indicator in the Public Health Outcomes Framework –funded from Public Health budget –responsibility of Local Authorities

Modernisation of health and social care - transition If programme transferred to LAs, need to identify key risks to the programme and mitigating action For both national and local attention Some risks will be generic – e.g. loss of corporate memory Others will be specific to the NHS Health Check programme – e.g. data retrieval from GPs Are asking for your participation – Heather White and Alison Daykin running a workshop
