Introduction Severe blood shortage occurs on a regular basis in South Africa The Problem: As a Level I Trauma Centre we want to eliminate and/reduce blood product wastage Conservation focus group started at Netcare Milpark Hospital part of Level I Trauma Centre commitment Focus areas: Trauma ICU and Burns ICU Role players: Trauma Programme Manager, ED Unit Manager, ICU Unit Managers, South African National Blood Services, Trauma Surgeons

Aim of Performance Improvement Focus Causes: - Cold chain management - Blood left in fridge post patient discharge (expired blood) _ Incorrect ordering (BRB boxes) - Amount of blood ordered Focus on amount of blood product wastage Focus on amount of blood products ordered and used Focused discussions: Trauma Management & Trauma Programme Meetings Decreased Wastage of Blood Products

Strategy for Change Cold Chain Management: –Transport and Storage Temperatures marked –Utilization of correct storage boxes for transport Documentation: Time of receipt of blood products, storage conditions of blood products improved Timely return of unused blood products Daily check on all blood products Training to staff

Implementation and Evaluation Monitoring of blood products - order vs. used vs. wasted Identification of causes for wastage Interventions and training to role-players to eliminate wastage Re-evaluation of effects of interventions and training during regular focused meetings Investigation of effects of change on cost P-D-S-A CYCLE Decreased Blood Product Wastage

Lessons Learnt & Message to others Main Focus Areas in the reduction of blood and blood products wastage: –Supply Chain Efficiencies: Ordering the specific amount and type of blood products to the point of issue of blood and blood products –Improving Health Provider Inventory Management: Receipt of blood and blood products to the transfusion of these units Timely return of unused blood products. Role players in the supply chain for blood and blood products have a responsibility to ensure that all aspects of the supply chain including transport, storage and inventory management minimise unnecessary wastage.
