3 Course Objectives 3 Describe the rule changes surrounding the use of the Certification of Services. Identify when the Certification of Services should be submitted. Complete Part 1 of the Certification of Services. Complete Part 2 of the Certification of Services. Identify resources to aid you with the Certification of Services.

4 What is the Certification of Services? Submitted with certain payment requests to ensure that Ticket Holders are receiving the services specified in the Individual Work Plan or IWP Addendum Request for Quotation/Codicil o At designated intervals in the payment cycle, the EN shall attest to having provided services to the beneficiary by completing and signing the EN Services Certification Statement available from the Operations Support Manager (OSM). The EN shall attach the EN Services Certification Statement to the EN Payment Request Form for Phase 1 milestone 4, Phase 2 milestone 11, outcome 11 and outcome 22. 4

10 Part 1: Statement of Services Provided 10 X 10/15/11 Submitted copy of beneficiary’s resume to ABC on 11/1/11. Scheduled interview on 11/15. List the services provided and their dates Indicate the most recent plan for services and provide the date

11 Part 1: Statement of Services Provided 11 beneficiary with monthly metro vouchers to travel to and from classes at the local community Trained Beneficiary on Microsoft Office applications from 1/6/12 to 2/21/12. Provided List continuing employment supports and their dates

15 15 Completing the Certification of Services Both Parts 1 and 2 of the form must be completed. If only one part is completed, the payment will be denied. In the event the beneficiary cannot be located or requests no contact, the EN should document the attempted contacts
