2.  OT : Single Wirral Wide Service provided by WUTH for Health and Local Authority.  Physio : Service provided by WUTH  SLT: The Majority of intervention is provided by Wirral Community Trust with some additional input being purchased by schools from private companies.  CHAMS: Provided by Wirral & West Cheshire Community NHS Trust.

3. Current Strengths and Skills: Alex is achieving well academically and has a large group of supportive friends both within and outwith the school setting. He is keen on sport and has aspirations of competing in tennis within the Paralympic Games when in 2020 Primary NeedAims & OutcomesHow Aims will be met Due to his Spina- bifida Alex has no voluntary bladder control and currently wears pads within school For Alex to be able to perform self catheterisation independently within School and home within 6 months Adaptations to home and school environments. Liaison and Joint sessions with Continence Nurse to improve independence. Alex aspires to compete within the 2020 Paralympic games as a singles tennis player For Alex to have sufficient control and speed within his wheelchair to enable him to complete. Within the 2020 games Liaison with Wheelchair tennis coach. Provision of advice on improving mobility performance. Liaison with OT and Sports Wheelchair company to assist in acquiring most appropriate Wheelchair,

4.  With their reports therapist will provide SMART Aims for Children and young adults outlining what their intervention will be for the next 1-3 years.  When completing an EHCP not all therapy aims and goals will need to be included. Only those Aims which the family feel are vital to enabling the child or young person to move towards their aspirations. The report can be an appendix if desired.  Therapists should identify individual needs not base their intervention on current services or practices.

5.  L is a 13yr old boy who has been recently diagnosed with Growth Hormone Deficiency, he also has a diagnosis of Autistic Spectrum Disorder.  He struggles with social situations and has high levels of anxiety and episodes of depression.  He is obsessed with playing football tending to be the only topic he is comfortable talking about.  His Aspiration is to become a Professional footballer and he says at his EHCP meeting he will be the best in the world.

6. Lionel Messi: the worlds Best Football Player was Diagnosed with Asperger's Syndrome at age 8 and with Growth Hormone Deficiency age 13. Yet has still gone to win 4 Ballon d’or Trophies. 4 Champions League and 3 World Cup Medals!!!!
