Mid Staffs NHS Foundation Trust Public Inquiry The background The Inquiry followed on from an original report published in 2010 which described the poor care Its aim was “To examine the operation of the commissioning, supervisory and regulatory organisations and other agencies including the culture and systems of those organisations in relation to their monitoring role….” It was undertaken to learn lessons It did not seek to apportion blame to individuals It stated that no reorganisation was needed – all changes can be done within existing system

The Inquiry spoke about… A lack of basic care and poor standards A culture that didn’t support the staff and ensure good care was provided for patients A focus on targets, finance and achieving Foundation Trust status Clinical staff and management not working together A lack of clinical governance A reliance on positive information whilst ignoring the negative

Warning signs were present… The Foundation Trust application Patient / staff surveys Whistle blowing External reviews The negative culture No patient voice An inadequate risk assessment of staff reduction Self promotion rather than critical analysis

The recommendations The report sets out 290 recommendations – not top down pronouncements but by the “…engagement of every single person serving patients in contributing to a safer, committed, and compassionate and caring service” We need to demonstrate –Do we accept the recommendations –What we intend to do to implement –Publish report on progress (annually)

Government response “The health and care system must change. We cannot merely tinker around the edges – we need a radical overhaul with high quality care and compassion at its heart” Jeremy Hunt Health Secretary

Government Response The Government set out a five point plan to revolutionise the care that people receive from our NHS, putting an end to failure and issuing a call for excellence: Preventing problems Detecting problems quickly Taking action promptly Ensuring robust accountability Ensuring staff are trained and motivated

The Health and Care Trust response Discussed at Board development session Paper to Trust Board Workshop at Quality and Safety Committee Various staff engagement meetings Public engagement All of this will feed into a Trust action plan to take forward relevant recommendations
