Important Choices Aids to Good Practice- Child & Young People (YP) 1. Identify children/ YP who may be near the end of life 2. Gauge child/ YP and family understanding of their condition and future 3.Assess and plan the child and family’s holistic needs 4.Consider the use of an advanced care plan 5. Implement effective multidisciplinary team involvement 6.Provide ongoing support to formal/ informal carers 7.Ensure appropriate care is provided to those affected by the death

Is catheterisation still required ( intermittent/ supra pubic/urethral? Has appropriate training / competency been completed? Ensure essential hygiene / asepsis, minimal breaks in closed catheter system If catheter bag needed, position correctly using appropriate fixation device, below bladder and off floor. Empty when 2/3 full and change bag (5-7 days) Observe urine for abnormalities, i.e. amount, colour, offensive smell, presence of debris/blood, Ensure adequate fluid intake (as per fluid matrix) Consider bowel habits (constipation) Protection from Infection 1 Aids to Good Practice – Children & Young People (YP)

Ensure all equipment that is provided is stored appropriately Follow infection control guidance to ensure all equipment is decontaminated effectively and in accordance with manufacturers instruction Ensure all equipment is serviced according to manufacturers guidance and Trust policy. Ensure clinical supplies are stored in a clean, organised and logical facility. Comply with infection control audits as necessary Support and educate families and carers regarding good infection control measures within the home environment. Keeping Nourished, Getting Better Aids to Good Practice- Child & Young People (YP) Are the children/YP ready to eat (hands washed, positioned to eat)? Does the child/YP need help to eat and drink (including position)? Are appropriate feeding devises available to help the child/YP take adequate diet and fluids and feed independently? Can the child/YP access food and drink when they choose to? Are the children/YP offered choices of food and drink if they are fed orally? Does the child’s diet need to be monitored due to poor weight gain or obesity? Is diet and fluid intake monitored and recorded appropriately? Does the child/ YP need dietetic assessment and advice? If the child / YP is fed enterally, are all carers trained and assessed as competent to administer feeds according to best practice?

Staying Safe Preventing Falls Aids to Good Practice -Children & Young People (YP) Has the child/ YP got an underlying problem with increased risk of falls or fear of falling? Are the child’s falls expected as part of growth and developmental milestones? Is the child/ YP on medication that may cause or treat symptoms that may cause falls and has medication been reviewed? Has the child/ YP had appropriate risk assessment and access to specialist advice and equipment e.g bed and appropriate bed rails? Has the child and family got access to alternative support or adaptive techniques to minimise falls and stay safe? Does the child and family need minor or major adaptations to their home? Is the child’s YP environment safe and will it remain safe in future care needs? Are the child’s/YP needs met within all care settings to minimise risk of falls and maintain safety?

Ready to Go - No Delays Aids to Good Practice – Child and YP 1.Assessing for appropriate admission to service / care provision 2.Engage with children / YP and carers 3.Identify plan of care and reassessment of care needs including referral to AHP’s and MDT 4.Estimate date for reassessment or expected date of discharge from service 5.Write & up-date clinical assessment and plan of care/discharge plan Additional checks: Plan time of discharge from ward or service if appropriate Communicate/ access any medication needs via GP / Consultant Confirm transport & telephone communication available Access any discharge letters when referred to community service Ensure communication with AHP & MDT at discharge and ongoing as needed.

Your skin Matters Children and Young People (YP) SSKIN Surface: make sure your patients have the right support Skin Inspection: Early inspection means early detection. Show Children & YP, parents and carers what to look for. Keep children and YP moving every 2-4 hours if any immobility Incontinence/ moisture: Children & YP need to be clean and dry. Nutrition/ hydration: Help children and YP have the right diet and plenty of fluids
