3.  You don't have to take our recommendations. You're free to elect any plans you want... but there's a lot going on in the background of the Exchange to help you find the best plans. CarePlus Benefits Exchange uses your benefits "personality" — everything it knows about you — your age, gender, marital status, family size, health background, and concerns and needs — to find the best plans to protect your health and your wallet. 6.  On the “Introduction” page, you’ll find educational materials and instructions, including a look at the plans available to you before you get started. Click the “Get Started” button to start shopping for benefits. 6 Introduction

7.  In “My Profile,” you’ll answer questions about your lifestyle, dependents, and risk tolerance. Your answers help CarePlus Benefits Exchange create a personalized benefits portfolio – just for you. Profile 7

10.  Complete the Health Profile for yourself and your dependent(s). Profile – Health Profile 10

11.  Tell us about your preferences for types of plans and networks. Profile – Plan Preferences 11

12.  Tell us about your tolerance for risk. Profile – Risk 12

13.  You’ll also be asked to answer brief questions about finances that will further tailor your recommended benefits portfolio. Profile – Finances 13

14.  Finally, you’ll be asked to answer a few questions about your lifestyle and your concerns. Profile – Lifestyle and Concerns 14

15.  Once your profile is complete, you’ll review the factors on which your recommendations will be based. Click “View Your Recommendation” to view your recommended benefits portfolio. Profile Results 15

16.  Keep in mind that our recommended benefits portfolio is calculated based on your responses, but this is only a recommendation. You’ll select and enroll in the benefits you choose in the following step. 17.  At the bottom of your recommended benefits portfolio, select “Add/Edit These Recommended Plans” to start with your recommended benefits or “Start with an Empty Cart” to start with an empty cart. Shop for Benefits – Recommended Portfolio 17

23.  Things change in life – and often those changes can affect your benefits. Things such as:  Marriage  Divorce  Birth  Adoption  Change in Employment  Death of a Spouse or Child  You can initiate your changes in the Things Change section of your Exchange. 24.  Things Change is located on your post-enrollment homepage. If you have a life events request after you’ve enrolled in your benefits, you’ll go here to submit it. Click on the “Change Requests & Updates” or “Things Change” to continue. Things Change in CarePlus Benefits Exchange 24
