3  I want to improve my ability to provide more health screenings for the disabled children at school  I would like to have a couple different days where we can meet at one place in a specific area over a span of a couple days and provide any heath screenings or issues needed.

4  Step 1: Participate in post-training meeting to review, revise and endorse my action plan; compare with/update procedures manual as needed.  Step 2: Find area's large enough to hold screenings for at least 100 children in the area.  Step 3: Assemble supplies needed, and make sure you have doctor on call for anything serious.  Step 4: Create posters and post them all around the neighborhood, and the school, to let them know what is going on.  Step 5: Review who will be there at what times. Review Dates also.  Step 6: Conduct a practice session with co-worker in area, to make sure everything runs smoothly.  Step 7: Observe and provide feedback during practice session.  Step 8: Provide lots of games and other forms of activities on opening day to create popularity around the area, and create signup slots so they are filled with screenings.  Step 9: Have doctor walking around to all of stations making sure everything is running smoothly.  Step 10: At the end, make sure all children have been seen and provide feedback on how the day went.

5 When: July 13 th, 2014 Where: Lake Eola Park Why: Free back to school health screenings Who: All Orange county Elementary Schools students with disabilities Time: 9am-2pm Information: There will be small lunches provided for those who get health screenings. We will also be selling school supplies for any children that are in need of specific supplies. This is brought to you by Family Practice East on Lake Underhill Rd.

6  First, I went to my family doctor and talked to them about how I would go about creating this special event.  It took around one month to get everything placed and all the information.  I then went around to all the elementary schools and handed out flyers so they could get the information out to their parents.  I got some of my old elementary teachers involved from Azalea Park Elementary where they helped me the day of with all of the screenings, and help communicate with all the doctors and nurses.

7  Teachers who helped out: 10  Doctors who did screenings: 7  Nurses who assisted families: 14  Children who received screenings: 61  Number of Elementary schools: 4  All races from Hispanic, Caucasian, Haitian, Indian, and African American.

8  After the event was over, I was so happy that I actually got a chance to make a difference in my community. Although It was very difficult to get everything together, I had a lot of help from my family doctor, family, and former elementary school teachers that truly made this event happen. I am so thankful for everyone that loves to help make a change in our community and help kids who cannot afford medical care.
